• Name of person submitting the photo
• Phone number to contact submitter (if needed)
• Names of people pictured in the photo
• The occasion during which the photo was taken (for example, Church Picnic 1985)
• If sending multiple photos, please name each photo and connect the photo file name with the appropriate details.
